Job Description
ChristianaCare HomeHealth is looking for dedicated and experienced Licensed Practical Nurses (LPNs) to join our team. If you are passionate about providing quality care to patients in the community, this is the opportunity for you.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement patient-centered care plans and assist in discharge planning.
- Support patients in New Castle County, DE, working day or night shifts, one weekend per month, and 2 holidays per year.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
- Prior experience in home health care is preferred.
