Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients in a community setting.
- Execute care plans that are patient-centered and aligned with available resources.
- Contribute to discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
