Join us in providing skilled nursing care to geriatric patients in the community, implementing patient-centered care plans, and contributing to discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement patient-centered care plans consistent with available resources.
- Support the development of discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
