Genesis Health is seeking a talented Minimal Data Set Coordinator - LPN to join our dedicated team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process, ensuring the delivery of quality patient care and proper nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary RAI process, ensuring accurate completion, encoding, and transmission of the MDS.
• Develop personalized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• Active LPN license with prior experience in a healthcare setting.
• Proficiency in general, rehabilitative, and restorative nursing practices.
• Computer skills, including Windows applications; AccuMed experience preferred.
• Prior experience as an MDS Coordinator; knowledge of RAI/PPS processes.
• Current BLS/CPR certificate.
