Nurse-LPN MDS Coordinator

Job Description

We are looking for a dedicated Nurse-LPN MDS Coordinator to join Genesis Healthcare, a leading provider of healthcare services known for delivering high-quality care with compassion. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process, ensuring proper patient care and nursing center reimbursement by completing essential items on the Minimum Data Set and Care Area Assessments.

Position Highlights

• Direct the interdisciplinary RAI process and ensure accurate completion and electronic transmission of the MDS.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items to new team members.

Requirements

• Active LPN license in good standing.
• Prior experience in healthcare settings like hospitals or long-term care facilities.
• Familiarity with nursing practices, medical procedures, and regulations governing long-term care.
• Proficiency in Windows applications; AccuMed experience preferred.
• Previous MDS Coordinator experience and knowledge of RAI/PPS processes.
• Current BLS/CPR certification.
