Responsibilities

• Conduct in-home Physical Therapy assessments and provide direct patient care.
• Evaluate medical conditions, functional capabilities, and potential for rehabilitation.
• Proficient in EMR documentation, experience with Hello Note system beneficial.
• Maintain an unencumbered Physical Therapy license for New York.

Requirements

• Graduate of an accredited Physical Therapy program.
• Valid CPR certification.
• Flexible scheduling options available including full-time, part-time, and per diem.
• Competitive pay rates ranging from $70 to $90 per visit.
