Physical Therapist Requirements:

• Graduate of an accredited Physical Therapy (PT) program.
• Valid and unencumbered Physical Therapy (PT) license in New York.
• Skilled in conducting home visit assessments, offering direct patient care, and evaluating medical conditions and rehabilitation potential.
• Proficiency in EMR documentation (experience with Hello Note preferred but not required).
• CPR Certification.
