Physical Therapist Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Holds an unencumbered PT license for New York.
• Conducts home visit assessments, providing direct patient care and rehabilitation potential evaluations.
• Proficient in EMR documentation (experience with Hello Note is a plus).
• CPR Certification.
