• Conduct home visit Physical Therapy assessments and provide direct patient care.
• Document patient information accurately in EMR systems.
• Collaborate with patients to assess medical conditions, functional capabilities, and optimize rehabilitation potential.

Requirements

• Graduation from an accredited Physical Therapy program.
• Active Physical Therapy license for New York.
• Proficiency in EMR documentation.
• CPR Certification.
