Physical Therapy Specialist Requirements

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit assessments, evaluate medical conditions, functional capabilities, and potential for rehabilitation.
• Proficiency in EMR documentation; experience in Hello Note system is beneficial but not required.
• Hold a valid CPR Certification.
