Physical Therapist Requirements

• Completion of an accredited Physical Therapy program.
• Valid and unencumbered Physical Therapy license in New York.
• Conduct home visit assessments to evaluate patient conditions, functional capabilities, and potential for rehabilitation.
• Proficiency in EMR documentation, experience with Hello Note is a plus.
• CPR Certification.
