Job Description
We are looking for a dedicated and experienced Registered Nurse (RN) Field Nurse for Home Care to join our dynamic team at ChristianaCare. As a Full Time RN, you will provide skilled nursing care to geriatric patients in the community, develop patient-centered care plans, and ensure optimal patient outcomes. This position offers the opportunity to work day shift hours in the Newark, DE area, with additional on-call responsibilities.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community
- Develop and implement patient-centered care plans
- Create discharge plans for patients
Requirements
- BSN required; ADN candidates must obtain BSN within three years of employment
- Current DE RN license or compact state license
- 2 years of recent HomeHealth RN experience
- Previous Home Healthcare and Med-Surgical experience preferred
- IV proficient or willing to train
