Job Description
ChristianaCare HomeHealth is seeking a Full Time Registered Nurse (RN) Field Nurse to join our team. You will be responsible for providing skilled nursing care and treatment to geriatric patients in the community, developing care plans, and ensuring patient discharges are well planned. This role requires professionalism, proficiency in IV administration, and the ability to work collaboratively with the healthcare team.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Develop and implement goal-oriented and patient-centered care plans.
- Plan for patient discharge ensuring continuity of care.
Requirements
- BSN required; ADN candidates must obtain BSN within three years of start date.
- Current DE RN license or compact state license.
- 2 years of recent HomeHealth RN experience.
- Proficiency in IV administration or willingness to train.
