Job Description
We are looking for a dedicated Registered Nurse (RN) specializing in Home Health to join the team at Atrium Health in Charlotte, North Carolina. As a Registered Nurse, you will be responsible for case management, providing direct patient care, and overseeing post-acute services in patients' homes. Responsibilities
- Manage an assigned patient caseload, delivering timely care and documentation.
- Collaborate with multi-disciplinary teams to ensure quality patient outcomes.
- Provide direct patient care and supervision of LPNs and HHAs according to care plans.
- Coordinate care with healthcare providers to meet patient needs and ensure high-quality outcomes.
Requirements
- Graduate from an accredited School of Nursing, BSN preferred.
- Current RN license or temporary license as a Registered Nurse Petitioner.
- BLS certification required.
- Valid driver's license.
