Registered Nurse (RN) - Home Health - $32-58 per hour

Job Description

We are looking for a dedicated Registered Nurse (RN) to join our team at Atrium Health. This position involves providing compassionate care to adult and geriatric patients in the comfort of their homes. As a Home Health Nurse, you will play a crucial role in overseeing and managing post-acute services, ensuring quality patient care and fostering positive outcomes.

Responsibilities

• Manage an assigned group of patients, overseeing and coordinating post-acute services in the home setting.
• Provide direct patient care and supervision of LPNs and HHAs according to established care plans.
• Coordinate care with a multidisciplinary team and act as the patient's liaison with other healthcare providers.
• Ensure timely care and documentation to meet patient needs and agency goals.

Requirements

• Graduate from an accredited School of Nursing, BSN preferred.
• Current RN license and BLS certification.
• Ability to prioritize and manage multi-issue situations effectively.
• Excellent communication and critical thinking skills.
