Responsibilities
- Conduct home visit assessments to evaluate patients' medical conditions and rehabilitation potential.
- Provide direct patient care and develop personalized treatment plans.
- Document patient progress using EMR systems.
- Ensure CPR certification and compliance with licensure requirements.
Requirements
- Graduate of an accredited Physical Therapy program.
- Valid Physical Therapy license in New York.
- Experience in conducting assessments and creating treatment plans.
- Familiarity with EMR documentation.
