Rehabilitation Therapist Requirements

• Graduation from an accredited Physical Therapy (PT) college or university.
• Active Physical Therapy (PT) license for New York.
• Conduct in-home Physical Therapy assessments, providing direct patient care to evaluate medical conditions and rehabilitation potential.
• Proficiency in EMR documentation, experience in Hello Note is beneficial.
• CPR Certification.
