Job Description

We are looking for a compassionate Rehabilitation Therapist to join Paradigm Rehab and make a positive impact on the lives of our patients. As a Physical Therapist, you will have the opportunity to help individuals reconnect with their environment and enhance their quality of life through rehabilitation.

Responsibilities

• Conduct home visit Physical Therapy assessments to evaluate patients' medical conditions and functional capabilities.
• Provide direct patient care, design treatment plans, and monitor progress towards rehabilitation goals.
• Utilize EMR documentation for accurate record-keeping and progress tracking.

Requirements

• Graduated from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license for New York.
• Proficient in CPR certification.
• Experience with EMR documentation is a plus.
